About the Assessment

Becoming an Anticoagulation Center of Excellence (ACE) provides your service with an invaluable opportunity to work as a multidisciplinary team, evaluating your current safety practices, and identifying areas for improvement. This comprehensive self-assessment is designed to assess your service against the latest standards, enabling you to accurately and honestly pinpoint specific areas for enhancement. By doing so, you will be equipped with targeted resources and support to elevate your practice to the highest level of care.

Enrollment and Process

Enrolling in the ACE Assessment is straightforward. By completing a brief registration process, you will gain access to the assessment. The benefits include the ability to benchmark your service against rigorously researched examples of top-performing services. This comparison will help you identify if your practice meets expectations and where improvements can be made.

Participation and Team Involvement

For the most accurate and comprehensive evaluation, only one representative from your service (ideally the Medical Director, Program Manager, or a designated leader) should complete the assessment. However, it is crucial to involve a team of practitioners who have a significant influence on anticoagulant patient care. We recommend convening this team to discuss your service’s current systems and practices thoroughly. This collaborative approach ensures that the assessment reflects a consensus view and is based on a thorough investigation of your service's performance.

Assessment Details

The assessment typically requires 30 to 60 minutes to complete. You may need to pause the assessment to consult with your team or gather additional information. You can save your progress and return to complete the assessment at any time. Upon submission, your assessment will be scored immediately, providing you with prompt feedback on your performance.

Ongoing Excellence and Renewal

Recognition as a Center of Excellence is valid for three years. To maintain this status, you must renew your recognition by retaking the assessment triennially. This ensures that your service remains at the forefront of anticoagulation management, continually meeting the evolving standards of excellence. By participating in the ACE Assessment, you commit to positioning your service as a leader in anticoagulation.
