These websites are a significant resource for guidelines and policies designed to assist in all phases of anticoagulant and INR management, clinic administration, and staff and patient education.


Clot Connect Clot Connect is an education and outreach project of the University of North Carolina at Chapel Hill Blood Clot Outreach Program. Clot Connect's mission is to increase knowledge of blood clots and clotting disorders by providing education and support resources for patients and health care professionals.
Institute for Systems Clinical Improvement ICSI promote various comprehensive guidelines and protocols which include Antithrombotic Therapy, Venous Thromboembolism, and VTE Prophylaxis.
IOWA Healthcare Collaborative The IHC has developed the following Anticoagulation Toolkit based on National Quality Forum recommended policies and procedures. IHC has compiled a vast array of forms, protocols, and guides for complete AC clinic management.
Society of Hospital Medicine – Venous Thromboembolism Resource Room The VTE Resource Room provides a detailed framework for establishing AC management in hospitals.
University of Michigan Cardiovascular Center Anticoagulation Management Service for Health Professionals This website provides specific Warfarin dosing and INR management guides. Patient Resources include ‘Vitamin K Content of Common Foods’, and ‘Living with Anticoagulation’.
University of Washington Medical Center – Anticoagulation Services This medical center, operated by the UW Dept. of Pharmacy, provides guidelines for numerous antithrombotic agents, and dosing corrections. They offer many patient education tools, some in multiple languages.
